This new guidance aims to highlight human factors involved in industrial accidents to help managers reduce incidents caused by human error. Aimed at managers with health and safety responsibilities, this guide encourages organisations to consider four areas of activity: during risk assessment; following an accident or incident; when designing or procuring equipment; in areas of health and safety management such as fatigue, communications and culture. A series of case studies is included.
